The idiom ‘not playing with a full deck’ means that a person is not mentally sound. It also means that a person lacks intelligence, but this meaning is used less.
It’s important to note that the phrase is derogatory.
The deck being referred to in the idiom is a deck of playing cards.
It is believed that the phrase was coined by the famous American stand-up comedian, George Carlin. At a show in 1965, he commented ‘I think we all realize he’s not playing with a full deck, folks.’ The comment got a huge reaction from the audience and since gone on to become a common idiom.

1965年に‘I think we all realize he’s not playing with a full deck, folks.’ （彼は精神的に健全ではないって私たちは気づいていると思う。）という形で、有名なアメリカのスタンドアップ・コメディアンのGeorge Carlinがいったことがはじまりです。そのコメントに対する聴講者からの反響が大きく、それ以来一般的に使われるイディオムとなりました。
Idiom Usage: Not Playing With A Full Deck 使い方
This idiom can be used by putting a subject and the ‘be’ verb in front of the phrase.
He is not playing with a full deck.
This idiom is usually only used in the negative form, but it is possible to use it the question form. As well, the idiom is mostly used in the present form. The ‘be’ verb can be altered to use the phrase in the past.
This idiom is more appropriate in casual situations; however it is sometime used in more formal circumstances to talk about a third party that’s not present. It would be very offensive to directly tell someone they are not playing with a full deck.
このイディオムは主語とＢＥ動詞を前において使われます。
He is not playing with a full deck.
また否定形の形で使われることが多いですが、疑問形として使われることもあります。なお、現在形で使われることがほとんどですが、ＢＥ動詞は過去形にかえて使われることもあります。
カジュアルなシチュエーションで使われることが一般的ですが、そこにはいない第三者の機関について話すようなときにちょっとしたフォーマルな場でも使われることもあります。直接的に相手にむかって、「they are not playing with a full deck.」というのはとても攻撃的な発言になります。

Use mnemonic! ニーモニックテクニック
Imagine trying to play card game, like poker, but the deck of cards you have only has 41 cards (a standard deck has 54 cards). The game is not going to be normal and can’t be played correctly. A person not playing with a full deck is not normal.
